Following its creation, the Great Waters Association of Vexillology solicited its membership for a flag design. The designs which were submitted were circulated to the membership and the flag design of John Purcell was selected as the association flag. It featured a blue field representing the Great Lakes and Ohio River and four chevrons, one each for Indiana Kentucky, Michigan and Ohio.
The flag was modified in June 2000 when Illinois was included as a part of the GWAV territory.
